The shared decision making platform is a web-based tool for people with chronic illnesses to track their symptoms, treatments, be able to easily share that day-to-day data with their health care provider. That data, and an appointment preparation tool makes this platform a robust point of reference for both patient and provider.

This product was created based on the need for a person with eczema, but with the long-term goal in mind that the platform could easily shift to track different illnesses. Which meant that the designed structure and navigation, as well as the tools and interactions, needed to be agnostic to specifics but with room to shrink or grow; flexible for more or less information, and visually versatile for varied uses.
